The Health Department is located in the basement of the Municipal Building.  The Health Department office is open Monday through Friday from 8:30 AM to 4:30 PM.

Influenza Update
According to the Centers for Disease Control and Prevention (CDC), flu activity remains relatively low
 (although it may rise or fall ) in most parts off the United States. Flu activity caused by either H1N1 (most flu continues to be 2009 H1N1) or seasonal flu viruses is expected to continue for several weeks in the United States. CDC recommends influenza vaccination as the first and most important step in protecting against the flu. Anyone who wants to be protected against the flu should consult their own health-care provider to receive the vaccine.  

Mass Dispensing Area 36 (MDA 36): City of Middletown, towns of Cromwell, Durham, Haddam, Middlefield, will not be offering any flu clinics at this time.  If you have any questions regarding H1N1 or seasonal influenza, please call (860) 344-3482.

Please note that you must apply for your marriage license in the town where the ceremony will take place.

Effective October 1, 2009 the fee for the following will increase.

Marriage Certificates $30.00

Marriage Certificate Worksheet

Note: All marriage certificates must be requested in person.  However, you can complete the Marriage Certificate Worksheet online, print it and bring it to the Health Department.  You must bring your social security card or a legal document containing you Social Security Number.  This will help us process your request more quickly.

To request a certified copy of your marriage license, send a letter to the Health Department with your names (bride and groom) and date of marriage.  Enclose a $20.00 check (no cash please) per copy requested.
